Endowment Fund Forms

Designated Endowment Fund Grants

Beneficiary agencies must submit a grant request form complete with the stated purpose and required signatures. 

All Designated Endowment Fund Grants must be approved by the HCCF Board of Directors. 

All Designated Endowment Fund Grants require a Final Report.

Donor Advised Fund Grants

The Founding Contributor or Designated Appointee must submit a grant request form with the stated purpose, name of the recipient agency, and required signatures. 

After the Founding Contributors or Designated Appointees have submitted the recommendations, all Donor Advised Fund Grants must be approved by the HCCF Board of Directors. 

All Donor Advised Fund Grants require a Final Report.
